Can you refer a person to The Living Hope House?
The Living Hope (TLH) House operates a robust referral process designed to offer you, as a potential referrer, your client, and us, real confidence.
We provide safe, supported accommodation for men rebuilding their lives after homelessness, addiction, or personal crisis.
Our homes are safe, structured, and grace-filled environments where people are supported to rebuild confidence, develop independence, and move from surviving to thriving.
Our Referral Process
Our referral process takes place in two stages to ensure we can provide the right support for both the individual and our existing residents.
Stage 1 – Online Referral
Complete our simple online referral form using the button below.
This provides us with the initial information needed to determine whether The Living Hope may be an appropriate service.
Stage 2 – Assessment and Interview
If the initial referral appears suitable, a member of our Support Team will arrange an assessment and interview with the prospective resident.
This enables us to gain a fuller understanding of the individual’s circumstances, support needs, risks, goals, and readiness to engage with supported accommodation. It also gives the individual the opportunity to understand how The Living Hope operates, including our clean and dry environment, house expectations, support programme, and community ethos.
The assessment helps ensure that we are able to meet the individual’s needs safely within our current service and that they are likely to benefit from the support we provide.
Following the assessment, the referral will be reviewed by the team and, if accepted, arrangements will be made for admission.
Our Support
Every resident receives a personalised support plan tailored to their individual needs and goals.
Using our secure Greenshoots case management system, our team works alongside each resident to record progress, review outcomes, monitor support, and celebrate achievements throughout their journey with us.
Support includes:
- One-to-one support sessions.
- Personalised support planning and regular reviews.
- Meaningful activity and practical work opportunities.
- Mentoring and pastoral support.
- Independent living skills.
- Move-on planning and preparation for independent living.
Our aim is not simply to provide accommodation, but to support lasting change and greater independence.
Who We Support
The Living Hope supports adult men who are:
- Homeless or at risk of homelessness.
- Recovering from addiction or personal crisis.
- Willing to engage positively with support.
- Able to live within a clean and dry environment.
- Working towards greater independence.
Every referral is considered individually to ensure we can safely meet the person’s needs while maintaining a positive and supportive environment for all residents.
Before Making a Referral
To help ensure The Living Hope is the right environment, prospective residents should:
- Have a genuine desire to make positive changes in their lives.
- Be willing to engage with support and work towards agreed goals.
- Be willing to live within a clean and dry environment.
- Be prepared to follow The Living Hope’s House Expectations and Licence Agreement.
- Be willing to participate in meaningful activities that support their recovery, wellbeing, and independence.
- Be committed to attending support sessions and working with the support team throughout their stay.
Participation in Christian activities is entirely voluntary. There is no requirement for an individual to declare or practise the Christian faith in order to access accommodation or support. However, residents should be comfortable living within a Christian community where our values shape the way we live and work together.
Meaningful activities may include volunteering, training, education, work experience, employment, practical projects, community engagement, or other agreed activities. As part of their support journey, residents are encouraged to engage in a minimum of 10 hours of meaningful activity each week.
